The table of contents is a reasonable representation of the table of contents from the original printing, although the page numbers in the original are separated from the titles by leader dots.

Although the table of contents lists an “Introduction” beginning on page 5, there is no section so titled, and page 5 contains the table of contents. It may be presumed that the introduction was reappointed as the preface, which appears on pp. 3-4 and is not listed, and the table of contents was not updated to reflect the change.
